Stroke Support & Information Service
NEW!
Carers’ Support (Bexley) has started a new service to support
Stroke Survivors and their Carers living in Bexley Borough.
The new service was launched in February 2008 and Barbara Cooper has been appointed as the Stroke Support and Information Worker.
When someone has a stroke people who have not previously been carers can suddenly find themselves in a frightening and bewildering situation. Practical help, information and emotional support are crucial needs at this difficult time for both the patient and his or her carer.
Barbara’s role includes liasing with carers whilst the patient is in hospital to offer immediate support and information and to help with financial assessment forms prior to discharge if this is requested. She will also be able to offer carers the chance to talk things through in private and provide emotional support and advocacy where this is also needed.
After the patient has been discharged Barbara will be able to visit families at home to offer on-going support, provide information about appropriate community services for both the carer and the cared-for person and refer families to these services. She will also be able to advise and assist with completion of Welfare Benefits forms.
Bexley patients may be admitted to Darent Valley Hospital in Dartford, Queen Elizabeth Hospital in Woolwich as well as Queen Mary’s Hospital in Sidcup. Barbara will visit all 3 hospitals on a regular basis to contact Bexley patients and their carers and to also develop links with key staff on the stroke wards.
The service is free of charge and self-referrals are welcome.
